- AT CHAPTER MEETING: Attend a meeting and ask for a member application at the check-in desk. Fill it out right there and give a check, cash or credit card number. Your card will come to you in the mail.

- AT CHAPTER MEETING: If you have not expired yet, pay the check-in person by check, cash or credit card number. If you have already expired, you will need to fill out another application and pay a $15 re-registration fee.

How to Change Your Address with FSMTA
- You can do it yourself. Go to www.fsmta.org and click on "Members Log-in" and click "View Profile" at the top. Enter your password. This will bring up your personal data. Scroll down to "Update Profile". Click and make changes to your file. If you have taken a workshop recently and want to add it to your profile, do it now. This is the official file of your information at FSMTA's Central Office. You have your own account within FSMTA, and you can make changes yourself.
- Secondly, you must notify your local chapter. Click info@fsmtasw.org to notify the Southwest Chapter of your current change of address to continue to receive bulletins from us.
Forgot your password?
All FSMTA members have an account with FSMTA
because of their membership and were assigned a password by email when
this service first went up. If you never received the password or lost
it, just click on "Members Log-in" and scroll down to "Don't know your
password?" Click here and it will ask you for your email address which
it will then cross-check with the data base, pluck out your password and
email it to you. In a few minutes you'll get an email from info@fsmta.org. Put this password where you put all your passwords so you can find it again.
You are listed on "How to Find a Therapist"
The Central Office data base links to a service called "How to Find a Therapist". This accesses the entire list of 4000-plus members statewide of FSMTA. By updating your information, potential clients can find you. All FSMTA members have an account because of their membership. The "Find a Therapist" page allows anyone to bring up a list of therapists by those modalities listed. You are limited to what is shown on the list, and cannot add an exotic-sounding workshop to your profile. For example, prenatal massage. When someone is looking for a prenatal massage therapist in their area, they will click on prenatal massage and enter their zip code and how many miles they are willing to drive to get to you, say 30 miles. If your zip code pops up within 30 miles as a prenatal massage therapist, your name and contact information will show up on the list. Try it now. It's pretty neat.
CEU's required every two years - next deadline: August 31, 2013
Massage Therapists need to have 24 hours of continuing education plus an HIV course every two years to renew their DOH license. Renewal date is August 31 on every odd-numbered year - 2009, 2011, 2013, etc. You may receive a notice in the mail but just in case you don't, be aware of the deadline. You can access the application online, shown below.

12 hours - Technique
2 hours - FL Laws & Rules
2 hours - Medical Errors
2 hours - Professional Ethics
6 hours - Massage Related courses such as communications with clients, insurance
reimbursement, charting, documentation, etc.
1 hour - HIV/AID's in addition to the 24 hours
24 hours total plus HIV/Aids
